Transferring a patient to Westfields Hospital & Clinic

At Westfields Hospital & Clinic, we can treat a wide variety of conditions in the majority of patients. To get started with a transfer or referral, doctors and clinicians can call the HealthPartners Direct 24/7 access center.

Patient transfer exceptions

Although we can treat most patients, there are a few exceptions. Patients who aren’t appropriate for transfer to Westfields Hospital include those needing:

  • Behavioral health
  • Burn Care
  • Cardiac catheterization
  • Critical Care
  • Hemodialysis
  • Interventional radiology
  • Neurosurgery
  • Pediatrics

How does the patient transfer process work?

At HealthPartners Direct, our specially trained staff members facilitate hospital admissions and emergency department transfers. When you call, we’ll ask a few questions about the patient and referring provider, then connect you with one of our clinicians. If necessary, we’ll arrange a safe and timely patient transfer.

Necessary patient transfer information

We’ll need the following information to connect the referring provider to the services the patient needs. Please have this information ready when you call HealthPartners Direct. (Note: We may ask additional triage questions, depending on the patient’s diagnosis.)

  • Caller name
  • Callback phone numbers (including direct number to referring provider, if different than the caller)
  • Provider first and last name
  • Patient demographics (including first name, last name, date of birth and medical record number)
  • Diagnosis and reason for transfer
  • Isolation precautions
  • IV drip medications (other than saline)
